We are thrilled that you wish to join our association. Whether you are applying for new membership or updating your existing membership data, this form is your first step to getting involved.

*Please note: only Indian citizens having attained 18 years of age and having benchmark Visual Disability (forty percent and above) can become a member of our association.

By becoming a member, you are joining a dedicated network of Disability Rights activists and volunteers committed to our shared aims and objectives. As a member, we ask that you regularly participate organization's activities to the best of your capacity and always be refrain from having regressive approach towards marginality of all forms.

*Blind Persons’ Association does not indulge in widespread malpractices across sectors; token social work is highly disregarded in our organisational value. We ask our members to perform their social responsibility and become an active citizen.

Instructions for filling out our membership form:

1. Download the membership form from here.
2. Complete all personal and contact details accurately.
3. Have a copy of your recent coloured passport-size photo ready.
4. Have a copy of your Disability Certificate ready to attach, as this is required to verify your percentage of blindness.
5. Have a copy of any conventional identity proof e.g. AADHAAR or PAN card ready.
6. Filled in form and photocopy of requisite testimonials along with annual membership fee of INR. 10.00 to be submitted in hand at our Central office at 6B Panchanantala Road, (2nd floor) Kolkata 700029.
